Completing a Form

You will find your forms listed under the “My Forms” tab in your profile. Forms listed under “Available Forms” are ready for you to complete. Some forms may be available for multiple submissions, while others will disappear from the list once you complete the form only once.

To fill out a form, click the pencil icon to the right of the form name. Form items with an asterisk (*) are require before you can submit your response.

To submit the form, click the “Submit Response and Finish” button at bottom right. Or, if you would like to save your work and submit the form later, click the “Save Current Progress and Finish Later” button at bottom left. When you do, the form will appear in the list under “Forms I started but have not yet completed.” To complete the form, click on the pencil icon.

All of your submitted forms will appear in the “Forms I already completed” list. To view your submitted form, click on the magnifying glass (if available). In some cases, you may also be able to edit a form after you have submitted it.

You will also see a list of “Forms others have assigned to you.” These are forms that others have complete for or about you (an evaluation form, for instance). If the form is viewable by you will see a magnifying glass icon to the right of the form. If the form is editable, you will see a pencil icon. If you see a pencil-in-a-box icon, this means that the form needs to be signed by you. Click the icon and look for the digital signature field, usually at the bottom of the form. There are four kinds of signature fields: a check box, a place for your initials, username and password, or your actual signature.

Once you sign the form, submit your signature by clicking the "Sign the Form" button.
